At SwiftMD we are committed to safeguarding the confidentiality and privacy of your Protected Health Information (PHI). We understand that your health information is very personal, and value your trust in sharing this confidential information with us. The medical record we maintain for you, and add to each time you receive medical care from SwiftMD, is the foundation for providing you safe and informed telemedical care.

All SwiftMD systems and processes comply with the standards set forth in the Health Insurance Portability and Accountability Act of 1996 (HIPAA). From our call center to our physician consultations to other member services, protecting your privacy and the security of your health information is a top priority.

Limited Access: We strictly limit access to your personal information. Only those employees and authorized agents of SwiftMD who need this information in order to provide you safe telemedical care have access under strictly controlled conditions. Employees who fail to follow our strict guidelines and protocols for safeguarding your personal information are subject to disciplinary action, including termination.

SwiftMD software and member health records are stored on secure servers and backed up daily. All data is encrypted during storage and transmission.

We will never communicate Protected Health Information, such as the results of diagnostic tests, to your personal or business email addresses. Instead, you can check those communications within our secure and private Member Portal at www.mySwiftMD.com. We will email you to let you know that a message is waiting in your SwiftMD In Box, and we will let you know if the message is Urgent or Non-Urgent.

SwiftMD never sells personal information we obtain from you to third parties for marketing or other purposes. However, we may share aggregated, non-personal information in a form that does not permit you to be identified, such as our total patient numbers, types of conditions being treated, and statistics on members such as patient age, demographics, etc. We may also share your information with partners in order to plan, market, and deliver new services and tools that can improve the health services that we can offer through SwiftMD, and for important research into the efficacy of telemedicine.

Medical Requirements to Share PHI: Some legal uses of your health information have to do with your medical care, or to protect the public health. These may include (1) using it for treatment of your condition and sharing the information with a laboratory or an imaging facility to perform appropriate tests, and/or your family physician or a specialist for purposes of further treatment; (2) using it inside our organization to assess outcome, improve quality and decrease risk of our services; (3) notify your family member, or anyone responsible for your care, of your location or general condition if necessary or requested by you; (4) notify emergency medical services or 911 of your location and condition if the circumstances of your call warrant an immediate emergency intervention; (5) as required by law, disclose your health information to public health or legal authorities charged with preventing or controlling injury or the spread of deadly or epidemic disease.

Legal Requirements to Share PHI: We may be legally required to release personal information to third parties, for example to comply with a law, regulation, medical release signed by you, search warrant, subpoena or court order. In the event that we are legally compelled to disclose your personal information to a third party, we will make every reasonable effort to notify you, unless doing so would violate the applicable law or court order. We may also release personal information in the event of an imminent threat to you or others, a threat to national security, a threat to our system or network, or cases in which we believe it is reasonably necessary to investigate or prevent harm, fraud, abuse, or illegal conduct.

Business Requirements to Share PHI: Authorized agents and contractors sometimes have access to your personal information in the course of assisting in operating our business and providing products or services to you. These contractors include vendors and suppliers that provide us with technology, services, and/or content for the operation and maintenance of our site. All contractors and authorized agents have a specific obligation under their contracts with us to keep your information confidential and to comply with our strict privacy and security policies.

Your Personal Password: Your cooperation in safeguarding your personal information is also important. Your password is critical to protecting your Personal Health Information. Choose it carefully, making sure it's easy for you to remember but difficult for someone else to figure out. It is also important that you remember to change your password frequently and never give your password to anyone.

Policy Updates: We reserve the right to update this Policy. We will notify all registered users of any material changes to the Policy. Our current Policy will be posted on both the SwiftMD Web site and secure mySwiftMD member site. By using this site and the SwiftMD services, you signify your acceptance of the SwiftMD Privacy Policy. If you do not agree to this policy, you should not use our services. We recommend that you read this Privacy Policy each time you use our services in case you missed our notice of changes to the Privacy Policy. Your continued use of the services and site following the posting of changes to these terms will mean you accept those changes
